Checking the client's identification band is the safest way to verify a client's identity because the band is assigned on admission and should not be removed at any time (if the band is removed, it must be replaced.) Asking the client's name or having the client repeat his name would be appropriate only for a client whos alert, oriented, and able to understand what the nurse is saying, but it isn't the safe standard of practice.
