The correct answer to this question is A, B, and C. A change model should include the steps that should be taken to handle the change, the responsibilities of each person including who should do what and what to do in the case of escalation, and time scales and thresholds for completion of the actions.

Unlike these pieces of information, complaints procedures are not included in a change model. A change request is when a client submits a proposal requesting an alteration to some product or system, or an addition or alteration to the agreed-upon deliverables for a contract or a project.
