The cause of one or more incidents is correct because it best describes a problem.
A problem as defined by ITIL is the unknown underlying cause of one or more incidents. A known error for which the cause of resolution is not yet known does not describe a problem.

The cause of two or more incidents does not describe a problem. A serious incident that has a critical impact on the business does not describe a problem.
