Option C - Deficient knowledge (disease process and treatment regimen). The nurse diagnosis that states the client action is deficient knowledge. Clients with pulmonary tuberculosis are no longer out in isolation. The treatment of tuberculosis has changed over the years.

The client does not seem to be anxious as he shows signs that he is upset rather than being anxious.
Social isolation is a wrong option, social isolation doesn't fit the client's action.

Impaired social interaction is wrong as this is more of a neurologic diagnosis not one that can be detected from the client's action or respiratory condition.

Deficient knowledge (disease process and treatment regimen)-rationale: this client is exhibiting deficient knowledgeabout the disease process and treatment regimen; treatment of tuberculosis no longer requires isolation, provided the client complies with the ordered medication regimen. although the client is upset, his question reflects sadness at the prospect of being separated from his family rather than anxiety about the disease. because he has just been diagnosed and hasnt had a chance to demonstratecompliance, a nursing diagnosis ofsocial isolationisnt appropriate. a diagnosis ofimpaired social interactionusually has a psychiatric or neurologic basis, not a respiratory one, such as pulmonary tuberculosis. client needs category: safe, effective care environment client needs subcategory: safety and infection control cognitive level: application reference: smeltzer, s.c., et al. brunner & suddarths textbook of medical-surgical nursing,11th ed. philadelphia: lippincott williams & wilkins, 2008, p. 650.
