To establish reasonable goals of a long-term nature for education and life in the community-the main goal of the transition plan is to put scaffolded support in place for your childs success beyond school. transition plans include specific goals based on your childs individual strengths and needs, and the actions necessary (steps) to achieve the goals. they also name persons responsible for each step, as well as a time frame for completion and accomplishment. the teacher writing the goal will confirm involvement by any person named as having responsibilities in the transition plan - for example, ensuring the availability of a community support worker - before adding these to the plan.
