An incident report has many forms and names. These include the test defect report, the defect report, and the test incident report and so on. However, in the standard for software system, it is officially called an anomaly report. This report basically displays the differences between the expected and actual results.

There can be a number of reasons for this difference. It may be because of manual error, system configurations or due to deliberate 2manipulations. These errors need to be recorded and displayed with full detail in these anomaly reports. This will allow the security services and the management to view and fix the cause.
